Enhanced Care Management
Project Kinship’s Enhanced Care Management program provides intensive, person-centered care coordination for eligible CalOptima Health Medi-Cal members who are justice-impacted and navigating complex medical, behavioral health, and social needs. Participation is voluntary and provided at no cost to eligible members.
Our team works alongside participants to make health care easier to access and understand while coordinating services that support overall health and stability. Project Kinship provides pre-release outreach, warm-handoff coordination, and post-release Enhanced Care Management to help eligible individuals successfully transition back into the community.
Support may include:
- Comprehensive health and needs assessments
- Individualized care planning
- Medical and behavioral health appointment coordination
- Connections to primary and specialty care
- Medication education and support
- Medi-Cal navigation
- Substance use treatment and recovery referrals
- Behavioral health service coordination
- Housing and shelter connections
- Employment and education referrals
- Benefits and public assistance navigation
- Ongoing follow-up, advocacy, and care coordination
- Support managing chronic medical or behavioral health conditions
For individuals with higher levels of need, our team provides consistent follow-up, advocacy, and coordination to reduce gaps in care and help participants remain connected to treatment and supportive services.
