Corrections Cohort
The Corrections Cohort provides intensive care coordination for individuals experiencing frequent involvement with the justice system, including participants with four or more incarcerations within the previous 12 months.
Through partnerships with Orange County agencies and community providers, Project Kinship helps participants navigate complex systems of care and connect to the services they need to build greater stability.
Support may include:
- Intensive case management
- Dedicated peer support
- Individualized care coordination
- Reentry planning
- Connection to behavioral health services
- Housing and shelter navigation
- Benefits and health care connections
- Substance use recovery support
- Community resource navigation
- Ongoing advocacy and follow-up
- Coordination with County agencies and service providers
Our Peer Navigators and case management team provide consistent, relationship-based support designed to reduce barriers, strengthen engagement, and create opportunities for meaningful change.
Through consistency, connection, and trusted relationships, we help participants break cycles of incarceration and build a more stable path forward.
